چکیده

he general assumption among cardiologists is that myoardial ischemia occurs only in the presence of obstructive oronary artery disease. This is based on the misconception hat abnormal coronary flow reserve (CFR) is associated nly with coronary stenosis and results from the increased esistance offered by the stenosis. We showed some time ago hat when the normal coronary vasculature is maximally ilated with adenosine, the capillaries (which do not have mooth muscle and hence do not vasodilate) are the bottleeck to hyperemic flow (1). Thus, although the capillaries rovide only one-quarter of the total microvascular resisance (MVR) at rest, they offer three-quarters of the total VR during hyperemia, despite the total MVR decreasing y one-half during hyperemia from arteriolar and venous asodilation.

Thyroxine-induced left ventricular hypertrophy in the rat. Anatomical and physiological evidence for angiogenesis.

We examined anatomical and physiological responses of the left coronary vascular system to thyroxine-induced myocardial hypertrophy. Wistar-Kyoto rats (1 and 5 months old) were administered thyroxine (0.25 mg/kg per day) or the saline vehicle (sham-treated controls) for 2 months. At the ages of 3 and 7 months, each group of animals was used for one of three experimental protocols: determination...

Decrease in coronary blood flow reserve during hyperlipidemia is secondary to an increase in blood viscosity.

BACKGROUND During maximal hyperemia, capillaries provide the greatest resistance to flow. A major determinant of capillary resistance is viscosity. We, therefore, hypothesized that abnormal coronary blood flow (CBF) reserve observed during hyperlipidemia is secondary to increased blood viscosity and not abnormal coronary vasomotion. METHODS AND RESULTS Maximal hyperemia was induced in 9 dogs ...

Evidence for decreased coronary flow reserve in viable postischemic myocardium.

To try to unravel the complexity and heterogeneity of the "no-reflow" phenomenon and its underlying mechanisms, we studied tissue perfusion in reperfused heart muscle by using tracer microspheres in an anesthetized dog model of 90-minute coronary occlusion followed by reperfusion for 2 1/2 hours, 24 hours, or 1 week. Regional myocardial blood flow was determined both in basal flow conditions an...

Role of capillaries in determining CBF reserve: new insights using myocardial contrast echocardiography.

To define the role of capillaries in the control of coronary blood flow (CBF) reserve, we developed a model of the coronary circulation and evaluated experimental data in its context. Our model comprised three compartments connected in series (arterial, capillary, and venous), each with its own resistance. The resistance in each vascular compartment was derived from the model based on hemodynam...

Fractional Flow Reserve and Coronary Computed Tomographic Angiography

Background: Invasive Fractional Flow Reserve Obstructive coronary artery disease (CAD) produces symptoms and increases the risk of adverse cardiac outcomes. However, determining whether a particular coronary stenosis is responsible for causing myocardial ischemia can be challenging, even with invasive coronary angiography (ICA). To help guide decisions in the cardiac catheterization laboratory,...
